Rethinking Musculoskeletal Diagnosis

Musculoskeletal disorders remain the leading cause of disability globally. Despite advances in imaging, surgical techniques, rehabilitation protocols, and manual therapy, recurrence rates remain substantial β€” particularly in low back, neck, and shoulder conditions.

Are we evaluating the full neuromusculoskeletal system β€” or primarily its structural components?

Traditional MSK assessment emphasizes:

However, they do not directly evaluate the system that governs movement: neural control.

Movement Is a Neurologically Regulated Process

The neuromusculoskeletal system consists of three interdependent subsystems (Panjabi, 1992):

The neural subsystem coordinates muscle activation, load distribution, proprioception, and segmental stabilization.

Emerging research demonstrates that altered motor control and delayed stabilizer activation are associated with:

  • Low back pain

  • Shoulder dysfunction

  • Knee instability

  • Recurrent injury patterns

Importantly, structural findings often do not correlate with symptom severity in asymptomatic populations.

This suggests that structural pathology may represent a downstream manifestation of altered load regulation rather than the primary driver.

The Diagnostic Gap

When neuromuscular control is not assessed:

  • Inhibition may go undetected

  • Compensation patterns persist

  • Structural correction may lack durability

  • Recurrence risk remains elevated

Without evaluating motor control, a specific functional diagnosis may be incomplete.
